Add a AUTHORS-additional file

Create a new file named ATHORS-additional and put in information on Fokus authorship due to changes made for Phoenix. This allows to leave the original AUTHORS file unchanged which makes keeping in-sync with the upstream easier.

Note: the AUTHORS file has been reverted to the original state as in github/master.
