Video/Audio quality is not initialized when ABR is off.

The quality is not initialized correctly when a new manifest is loaded and the ABR is off. Here is how you can reproduce the issue:

1)  Make ABR off
2)  From the Stream pull down menu choose 'Envivio' (first item).  Click Load button.
3)  Allow it to start and change the video rep. index to 2 by clicking '+' next to video menu
4)  From the Stream menu choose 'DASH-AVC/264 - test vector 4b - Sony' (last item). Click Load.
5)  The video does not load.
