Low latency improvements

Improved general stability of the low latency mode by fixing the time offset calculation (by removing the miliseconds from the sum, because the source lacks them), adjusting the stall threshold and retry mechanisms.

Also improved the client throughput calculation when using fetch, leading to a better ABR performance in low lat streams.
