#r time fragment not limited by stream start

The #r parameter introduced in #2403 only works when the DVR window doesn't go earlier than the start of the stream, but it should also be limited by beginning time of stream. This fine for a long running stream, but if the stream has just started or the DVR window is very big, this won't work.
